Appointments
No appointment is necessary, but this will change by summer of 2002. We do suggest you call ahead to make sure that Dr. Taylor will be available to see patients.
All walk-ins are welcome!
Your First Visit
You will be asked to fill out a New Client Form that will allow us to create a personal record of you, and all of your pets, in our computer system. This will enable us to remind you whenever vaccinations, medications, or hospital procedures are due for your pet. Also, dependent on the situation, you may be asked to fill out additional forms that will help Dr. Taylor in diagnosing your pet's condition. This information is very important, so bring the medical records that you have. If you do not have the information on hand contact your previous veterinarian and have them fax it to our office. Deposits
Deposits are required before any hospital procedures, or hospital admissions. Deposit amounts depend upon the treatment being given. CARECREDIT applications are available upon arrival or by fax.
Discharging Patients
All patients must be picked-up during regular business hours. No surgical patient will be discharged any earlier than 9am the day after surgery.
Mon., Tues., Wed., and Fri. 8am - 5pm
Thur. & Sat. 8am - 12pm
Surgical Admissions
Surgical patients must be at the hospital between 8am and 9am the morning of the surgery. Surgeries are only performed on Mondays, Tuesdays, Wednesdays, and Fridays. It is hospital policy, that all surgical patients must stay for 24 hours. We recommend that you stop feeding your pet at 8pm the night before surgery. You may leave water for them to drink. Please allow approximately 15 minutes to fill out consent forms. We STRONGLY suggest that you have preanesthetic blood work done before every surgery. This will let Dr. Taylor know if there are any preexisting conditions that may complicate the surgery. Also, this will give us a record of your pet's normal ranges in case a serious illness occurs in the future.
Consent
We cannot guarantee the success of every medical procedure; however, that is the ultimate goal at Southwest Animal Hospital. Due to this fact, you will be asked to sign a consent form which will assert that you understand, and agree, that unforeseen conditions or complications may arise while your pet is in our care. Please read this form carefully before signing it. If you have any questions before signing this form please feel free to ask one of our staff members to explain it in further detail.
Emergency Admissions
In case of an emergency please call our office immediately. Because we have one doctor, there will be times that he is unavailable to take emergency patients. Please feel free to contact another emergency care facility. During business hours, our staff will be able to assist you with any issues related to your emergency.
Any emergency that requires after hours admission, or care, will incur an emergency fee in addition to all other charges. A deposit will be required at the time of admission.
